How much does FDA approval cost for a medical device? There is no single sticker price: FDA clearance or approval is a basket of expenses spread over many months — a user fee paid directly to the agency, third-party laboratory testing, building a quality system and documentation, and the working hours of regulatory specialists, engineers, and technical writers. As an honest order of magnitude, a simple 510(k)-track device usually lands in the tens to low hundreds of thousands of dollars all-in, while PMA-track devices run far higher. The point to internalize early: the official FDA fee is usually the smallest component of the bill. This article breaks the cost into its parts so you can build a budget that holds. User fees: what you pay the regulator

  • Submission fee. A one-time fee for review of your file, scaled by submission type. A 510(k) sits at the low end of the scale, De Novo considerably higher, and PMA — the pathway for the highest-risk devices — an order of magnitude above that. The amounts are updated every fiscal year and published on the FDA's website, so always check the current fee table before budgeting.
  • Establishment registration. An annual fee for being a registered device manufacturer, payable for as long as the device is marketed. It is a recurring expense, not a one-time cost, and it is routinely forgotten in financial models.
  • Small business status. Companies under a defined revenue threshold can apply for Small Business Determination and pay a substantially reduced submission fee. The application is separate, requires financial documentation, and takes time — start it months before you submit, or you will pay full price.

Laboratory testing: the line item that surprises almost everyone

Most of the approval budget goes not to the regulator but to accredited third-party test labs, and the scope is driven directly by device type and body contact. A device touching intact skin needs one biocompatibility panel; one contacting blood or tissue needs a far larger battery. An electrical device adds IEC 60601 electrical safety and EMC testing. A sterile device adds sterilization validation, packaging validation, shelf-life, and accelerated aging studies. A device with software adds software documentation, verification and validation, and cybersecurity requirements. Each test is priced separately, and the testing total frequently exceeds the user fee several times over. Quality system and documentation: the cost that is hard to see

The FDA does not evaluate only the device — it evaluates how you designed and manufactured it. Budget-wise that means investing in a quality management system aligned with the FDA's Quality System Regulation and ISO 13485: procedures, design controls, risk management, verification and validation protocols, change control, and traceability from requirement to test. The cost is a mix of consulting hours, your team's internal hours, a documentation system, and often an external audit — and it continues after clearance. Budget it as an operating expense, not a project that ends on submission day.

Consultants, testing strategy, and the second round

  • Regulatory consultant. Most first-time teams hire one to write the submission and manage dialogue with the agency. Pricing ranges from hourly to fixed-fee, and the gap between quotes is wide — define up front what is included, especially whether responding to an FDA Additional Information request is covered or billed separately.
  • The AI request round. A large share of submissions receives a request for additional information. That means more work hours, sometimes supplemental testing, and months of delay to revenue. A budget without reserve for this round is not a realistic budget.
  • Technical writing. A submission is an engineering document written in precise regulatory English. Amateur writing generates questions, and questions cost money and time.

How to build a budget that holds

Three rules prevent most surprises. First, pin down classification and pathway as early as possible — they dictate every other number. Second, build the test plan during design, get lab quotes before locking materials and suppliers, and plan a dedicated prototype build for testing. Third, remember approval is not the end of spending: annual registration, quality system maintenance, and post-market surveillance follow.
